What is Case Management? Case Management is a confidential and professional service offered to people with disabilities. Case Management is an individually centered, family and community focused process through which consumers are assisted in identifying, securing and sustaining resources necessary to live in a normally interdependent way in the community. Eligibility The applicant must be eligible for Title XIX and have a diagnosis of Chronic Mental Illness, Mental Retardation, or Development Disability. They must want and need coordination of service. Children under the age of 18, must have a diagnosis of Mental Retardation or Brain Injury and be accessing ether the HCBS/MR Waiver, or the HCBS/BI Waiver. Application Information Anyone can request case management services for themselves or for someone else by contacting Allamakee County Case Management, 110 Allamakee Street; Waukon, Iowa 52172. Upon request, a case manager will meet with consumer and/or parent/ guardians to determine eligibility, and determine a plan for achieving goals. Accreditation The Allamakee County Case Management program is accredited by Iowa Department of Human Services, Mental Health and Development Disabilities Commission. Advisory Board An Advisory Board oversees the program and meets 3 to 4 times a year to discuss issues. Central Point of Coordination (CPC) The Case Manager will assist in filling out a CPC Application, Which is necessary to access county - Funded Services.
